Users must provide at least one (1) emergency contact and may provide up to three (3).

For each contact, we collect:

• Cell Phone number

By providing emergency contact information, you represent and warrant that:

• You have obtained permission from each emergency contact

• You have informed them that they may receive automated SMS notifications

• You are authorized to provide their contact information

• Daily check-in confirmations

• Missed check-in records

• SMS notification timestamps

• Device type and operating system

1. D. SMS Communications

If you miss a daily check-in:

1. You may receive:

“Cecelia, are you okay?”

2. If no response is received, your emergency contact(s) may receive:

“Cecelia has not responded to her daily Check On Me™ wellness check. Please reach out. This is an automated notification service and not an emergency response provider.”

SMS messages are delivered through Project Broadcast.

By subscribing, you expressly consent to receive automated SMS messages related to your safety notifications and account.

2. How We Collect Information

We collect information when you:

• Register for an account

• Subscribe to the App

• Enter emergency contact information

• Interact with the App’s daily check-in system

• Trigger SMS notifications through missed check-ins

• Deliver daily check-in notifications

• Send SMS escalation messages

• Process subscription payments

Users cannot delete accounts within the App.

Safety escalation alerts to your designated emergency contact(s) if you fail to respond

Message frequency varies depending on user activity, including missed check-ins and system-triggered alerts.

Message and data rates may apply based on your mobile carrier plan.

Opt-Out Instructions:


If you are receiving text messages from us and wish to stop receiving them, simply reply STOP or UNSUBSCRIBE to the number from which you received the message. You will not receive further messages unless you opt back in.

User Responsibility for Emergency Contacts:

By providing emergency contact information, you represent that you have obtained consent from each contact to receive SMS notifications from Check On Me™.

No Sharing of Mobile Information:
